2018 Webcast: Group Screening Through Value Based Management Principles
This 2018 Value-Based Care course will review the patient group orientation process from two unique perspectives through the lens of value-based care. During this session, attendees will learn about the rational for implementing group orientation, different models to deliver group orientation, and data and techniques for data collection as it relates to group orientation. This presentation will relate their data to the value based care model of increased health care utilization with a decrease in time to enrollment while decreasing health care costs through efficiency.

- Understand the group orientation model in which the establishment of the ITP and exercise assessment occurs.
- Understand the group orientation model in which introductory information is presented to patients to influence enrollment.
- Identify methods of data tracking with the implementation of group orientation.
- Correlate the group orientation model to Value-Based Care utilization and efficiency.
- Identify staff resistance to change and opportunities for engagement.
