This 2018 Value-Based Care course will review the process of patient referral and participation in cardiac and pulmonary rehabilitation through the lens of value-based care. During this session, attendees will learn how to and about reduction in health care utilization and cost, how to develop effective data tracking tools, and establish a rehab referral work flow.

This course will review the process of patient referral and participation in cardiac and pulmonary rehabilitation through the lens of value-based care. During this session, attendees will learn how to and about reduction in health care utilization and cost, how to develop effective data tracking tools, and establish a rehab referral work flow.
Learning Objectives
- Understand the value of engaging health system Clinician and Administrative Leadership to support the cardiac and pulmonary rehabilitation program.
- Describe methods to identify eligible inpatients and facilitation of their referral to outpatient cardiac and pulmonary rehabilitation programs.
- Develop a data tracking mechanism (referral and enrollment, patient outcomes and costs) to support the growth/development of the cardiac and pulmonary rehabilitation program.
