Collaborative self-management describes the combined effort of the health care team and the patient/family to meet the needs of the patient as they relate to management of the patient’s lung disease. This session discusses how Pulmonary Rehab programs can train patients to manage their chronic lung disease by guiding them to make behavior changes that could potentially lead to improved health.
