2016 Annual Meeting Recorded Session 112: Million Hearts Cardiac Rehabilitation Collaborative - Progress Made and Map to the Future
In November, 2015 over 20 professional organizations, The Centers for Medicare & Medicaid Services, along with several payer and patient groups came together to identify actions that will advance the utilization of cardiac rehabilitation services in the next 1-2 years. This session will review the purpose, progress, and goals of the Million Hearts Cardiac Rehabilitation Collaborative (CRC). This initiative will drive strategies considered critical to future change, growth, and sustainability of cardiac rehabilitation services.

Learning Objectives:
- Attendees will be able to explore and develop effective program-related strategies that target improved patient adherence to scheduled cardiac rehabilitation sessions.
- Attendees will be familiar with the evolution of the science that is the foundation for the current practice of cardiac rehabilitation.
- Attendees will be aware of the CRC “road map” that focuses on the potential impact of interventions to increase referral, enrollment, and adherence to cardiac rehabilitation.
- Attendees will be knowledgeable about actions currently underway to achieve CRC goals for increasing CR referral and enrollment.
