Million Hearts® has created clinical quality improvement change packages to help clinicians and health systems integrate evidence- and practice-based strategies into clinical care. The Million Hearts®/AACVPR Cardiac Rehabilitation Change Package (CRCP), launched in 2018, is being implemented nationwide to incease referral, enrollment, and participation in cardiac rehabilitation. In January 2020 the Agency for Healthcare Research and Quality (AHRQ) launched an initiative to help over 300 hospitals implement automatic referrals with care coordination, leveraging the strategies and resources from the CRCP. TAKEheart participants and subject matter experts will share what progress has been made and what initial lessons have been learned in year one of three. To wrap up, CDC subject matter experts will discuss specific strategies from the Million Hearts® Hypertension Control Change Package (March 2020 – 2nd edition) and the Million Hearts® Tobacco Cessation Change Package (September 2019), and describe the value these tools offer in improving cardiovascular health outcomes.
